Passengers requiring medical authorization to travel

Passengers who have a specific medical condition must present his or her medical documentation so that they may be reviewed by one of LAN's doctors.

The reason for this review and authorization is to ensure that there is no existing counter-indication, from a medical point of view, that may be harmful to the patient when traveling.

General information

Passengers requiring an authorization to travel may complete and turn in the Health Information Form (MEDIF).

This document consists of two sections which must be completed:

Section 1a must be completed by LAN's sales office or the travel agency that sold the ticket, or, in its absence, may also be completed by the passenger.

Section 2a must be treated by the physician who is providing medical treatment to the passenger*.

*To avoid delays in the authorization process, please make sure that all required spaces have been properly filled out.

Download the Medical Information Form (MEDIF). In order to be approved in time, once completed, the MEDIF form must be faxed over to the call center at 305-670-5501 or contact your travel agency at least 24 hours¹ prior to departure.

Who must complete the MEDIF form?

Exceptions:Passengers who are in a cast must present a MEDIF form only when it is an articulation fracture (ankle, knee or elbow). Other types of casts do not require a MEDIF form. However, they must present a health certificate at the moment of boarding, stating a diagnosis.
