It is of the utmost importance that passengers with certain medical conditions present their medical records so that these may be evaluated by a LAN doctor, who will verify their ability to travel by air.

The objective of this review is to guarantee that there is no reason, from a medical perspective, that suggests that the passenger should not be allowed fly.

This document consists of two parts that must be completed:

The first section must be completed by the travel agency or LAN sales office that issued the ticket. Failing this, it can also be completed by the passenger.
The second section must be completed by the passenger's treating doctor.

In order to download the Medical Information Form (MEDIF).

Once completed, the MEDIF must be turned in at a sales office or the relevant travel agency a minimum of 24 hours prior to the flight's departure in order to be approved by the LAN doctor.

Who needs to complete the MEDIF?

Newborns or unweaned infants that require oxygen or any kind of monitoring.
Newborns born within the last seven days.
Children requiring incubators.
High-risk pregnancies.
Women who have give birth within the last seven days.
Passengers requesting stretchers.
Passengers requesting oxygen.
Patients who have undergone surgery within the last 10 days.
Passengers with terminal cancer.
Passengers with Disabilities: Mental and Psychiatric.
